Cool Or Not Cool: Restaurants Charging a Delivery Fee?

Some airlines charge a fee for reserving a flight on the phone or in person, while a few moving companies have been known to tack "environmental fees" onto their bills. We've got to keep an eye on the fine print, otherwise someday we may end up unknowingly paying a "poop disposal" fee at the vet's office.
FiPS reader Lisa recently wrote in about a local joint that charged her a delivery fee.
I had no idea there was such a thing as a restaurant charging for delivery. Did you know about this? La Villa just charged me $1.50. Guess I'll be reading those delivery receipts a bit more closely in the future.
I don't think I've ever noticed a delivery fee on a receipt before, though I probably don't inspect them as closely as Lisa. Is this a common thing in the area? What does a delivery fee even cover: bike and scooter repairs?
I've always understood the idea behind having a delivery minimum of $10 or $15, but an additional fee seems like overkill when you're also tipping the man or woman delivering that piping hot goodness.
